According to "My Heritage" David passed on June 19, 1995, living in Centralia, Wa. Cause of death was not stated. He was born in Yuba City,  CA.

His Mother was Jacqueline J. Sutton Mateofsky & Brother Alexander J. Mateofsky, dob2/11/50.
